If you received a Grant from BWC previously, you already have a Supplier ID Number.
Steps to Apply for Safety Intervention Grant:
Create an account in the grant management portal. To request access to apply for a grant, you’ll need to provide your policy number. You can then approve yourself to apply by providing your organization’s EIN. This video will walk you through the process.
To start an application, select New Applicationon the left under My Applications.
Once your intervention(s) is determined, you’ll complete the questions on the online application in the grant management portal. A video of this process can be found here. I suggest filling in the template Julie from BWC Sends you and together you can finalize your answers before you go online! ( Julies Information at the bottom of page)
Someone with fiduciary responsibility must sign the Section V Budget and VI Statement of Agreement sections.
Submit the online application.
Number of affected employees.
List of Claims – check if any were related to the area for improvement
Description of current situation:
how often tasks are performed
basically, tell BWC how bad the situation is!
Description of intervention and how it works.
How the intervention will reduce or eliminate exposure.
How employees will be trained on new equipment.
Description of how any potential hazards created by the equipment will be addressed.
Time it will take to order and install equipment.
Transfer cost/freight/taxes directly from the quote onto the online application.
The quote must:
Be good for 120 days
Address of the vendor and your company contact
State that it is a QUOTE – not an invoice
Upload quote into the online application
Signed by Fiduciary Responsible Individual via e-signature.
Fast Track Items:
Cable pulling equipment
Chest compression systems
Conduit bending equipment
Cot loading equipment
Floor cleaning equipment
Flooring or floor treatments
Hose reel equipment
Mulch dispensing equipment
Patient lifting devices
Patient stair chairs
Portable traffic signals
Powered pallet jacks
Saws with presence sensing technology
Scissor lifts, vertical mast lifts
Snow removal equipment
Truck tarping systems
Equipment primarily necessary for the business (e.g., forklift).
Items that are not engineering controls (e.g., PPE).
Items that don’t directly reduce workplace risk factors (e.g., lighting).
Other items outside the scope of the program (e.g., insurance).
After Submitting Application:
Your application will be reviewed by either one or three individuals on the review committee
Pass or Fail
Pass – expect your funds to arrive in ~6 weeks
Fail – they will explain why and offer you the opportunity to change or add more information
After Receiving Funds:
Once you receive grant funds you must purchase the equipment and implement the intervention within 90 days of the date of the grant check.
Submit proof of spending within 120 days of receiving the funds
Provide the purchase date and implementation date.
Contact the BWC Consultant to schedule a follow-up visit.
Submit the One-Year Case Study within 90 days of the one-year reporting period.
